I found an adorable duvet cover and quilt at Nordstrom’s Anniversary sale but did not like the Euro shams so I bought two standard that match the duvet and two that match the quilt and added a cute rectangular accent pillow. They are gorgeous.

In Europe and Scandinavia, the duvet insert and its duvet cover are usually all that top the bed above the sheet-covered mattress, but you can use a sheet under the duvet, too.

I do quilts! HI Katherine, the pillows and duvet came from the Pottery Barn years and years ago! Mind you, I love how the bed looks when it's made, and I always want the bed made when company comes over, but I just don't like the act of doing it. It’s like a faux duvet cover, and the sheets can be a little easier to wash weekly than removing and washing a duvet cover (though real duvet fans know the small extra effort it always worth it). If you are using a feather duvet with a cover, take it off the bed and give is a few good punches with both hands. Even though I don’t like to iron bed linens I will plug in the iron next to the bed (put a fluffy towel under it) and iron the top part of the flat sheet. Or if you live in a part of the world that gets an evening chill, at least swap your heavy down comforter for a lighter, summer weight comforter like we did on the bed above.
